The long-term goal of antiviral therapy for patients with chronic hepatitis B is functional cure (HBsAg loss). However, achieving HBsAg loss with existing drugs such as nucleo(s t)ide analogues and PEG-IFN remains challenging in many cases. Currently, the development of new therapeutic agents based on the HBV life cycle is underway, and there is also anticipation for the enhancement of biomarker sensitivity and the development of new markers that contribute to the evaluation of therapeutic efficacy. On the other hand, in clinical practice, discussions continue regarding the risk of hepatocarcinogenesis and the indications for treatment in the\u201cgray zone,\u201dhighlighting the need for further evidence accumulation. In this session, we aim to broadly discuss the current state and challenges in HBV treatment, including the administration of existing drugs, the development of new treatments, and the simplification of diagnostic methods. As we enter the 2020s, the treatment paradigm for advanced hepatocellular carcinom(a HCC)has undergone a significant transformation with the advent of combination immunotherapy. Remark- able tumor shrinkage is now being observed in cases previously deemed incurable, leading to a steadily improving prognosis for patients. Moreover, advancements in systemic therapy have broadened surgical options, giving rise to the new concept of\u201cborderline resectable\u201dHCC. The global death toll from viral hepatitis ranks as the second highest cause of infection-related mortality, following tuberculosis, highlighting the urgent need for measures toward eradication. In antiviral therapy for viral hepatitis, complete elimination of the virus remains challenging in hepati- tis B, and new drug developments are being pursued. While hepatitis C has seen high rates of viral eradication, challenges persist in managing difficult-to-treat cases, and the risk of liver disease pro- gression and hepatocarcinogenesis remains even after viral clearance. New infections continue to occur for both viruses, making efforts toward eradication crucial. On the other hand, many aspects of hepatitis D, such as its prevalence and natural course, remain unclear, and no established treat- ment exists. Additionally, managing cases of acute hepatitis, including hepatitis A and E, that prog- ress to acute liver failure can be difficult. MASLD affects approximately 30% of the population in Japan. The prevention, diagnosis and treatment of complications of MASLD is therefore an important societal challenge but an unmet need. It is important to identify high-risk cases from the large MASLD population and to develop effective strategies. A new nomenclature of steatotic liver disease has been established and patients are stratified into MASLD, MetALD and ALD according to alcohol intake. However, the differences in pathophysiology and risk of complications between MASLD, MetALD and ALD need to be clarified. The efficacy of complication drugs such as GLP-1 and SGLT2 in the treatment of MASLD needs to be further clarified. However, achieving HBsAg loss with existing drugs such as nucleo(s t)ide analogues and PEG-IFN remains challenging in many cases. Currently, the development of new therapeutic agents based on the HBV life cycle is underway, and there is also anticipation for the enhancement of biomarker sensitivity and the development of new markers that contribute to the evaluation of therapeutic efficacy. On the other hand, in clinical practice, discussions continue regarding the risk of hepatocarcinogenesis and the indications for treatment in the\u201cgray zone,\u201dhighlighting the need for further evidence accumulation. In this session, we aim to broadly discuss the current state and challenges in HBV treatment, including the administration of existing drugs, the development of new treatments, and the simplification of diagnostic methods. Remark- able tumor shrinkage is now being observed in cases previously deemed incurable, leading to a steadily improving prognosis for patients. Moreover, advancements in systemic therapy have broadened surgical options, giving rise to the new concept of\u201cborderline resectable\u201dHCC. This symposium will spotlight the new era of advanced HCC treatment, delving into optimal strategies for combination immunotherapy, long-term outcomes, practical applications of conversion therapy, novel therapeutic approaches, maximizing synergies between locoregional and systemic therapies, treatment optimization based on real-world data, and the exploration of new biomarkers for per- sonalized medicine. In antiviral therapy for viral hepatitis, complete elimination of the virus remains challenging in hepati- tis B, and new drug developments are being pursued. While hepatitis C has seen high rates of viral eradication, challenges persist in managing difficult-to-treat cases, and the risk of liver disease pro- gression and hepatocarcinogenesis remains even after viral clearance. New infections continue to occur for both viruses, making efforts toward eradication crucial. On the other hand, many aspects of hepatitis D, such as its prevalence and natural course, remain unclear, and no established treat- ment exists. Additionally, managing cases of acute hepatitis, including hepatitis A and E, that prog- ress to acute liver failure can be difficult. The prevention, diagnosis and treatment of complications of MASLD is therefore an important societal challenge but an unmet need. It is important to identify high-risk cases from the large MASLD population and to develop effective strategies. A new nomenclature of steatotic liver disease has been established and patients are stratified into MASLD, MetALD and ALD according to alcohol intake. However, the differences in pathophysiology and risk of complications between MASLD, MetALD and ALD need to be clarified. The efficacy of complication drugs such as GLP-1 and SGLT2 in the treatment of MASLD needs to be further clarified. This symposium will encourage a broad call for the latest research in MASLD, from diagnosis to treatment, to address the current unmet needs.<\/p>\n <\/div>\n <\/div>\n <\/article>\n
